Treatement including acupuncture, electric acupuncture, laser acupuncture, cupping, moxa and manipulate therapy depend on the patient's condition.

I understand by signing this form that the information provided is true to the best of my knowledge. Changes to the above should be advised upon future visits. I consent to receive treatments after discussion of treatment and its benefits with the practitioner. By signing this form, I acknowledge that during the treatments there may be physical contact during massage, when inserting/removing acupuncture needles and during other treatments. I also understand that all bills are to be settled at the conclusion of each visit.
